Challenge
Prison Network was successfully managing its group programs in Salesforce but faced gaps in how it recorded one-on-one support for women and families. Information was stored inconsistently, making it difficult to track and report on individual engagement.
Life circumstances changes, such as parole, custody status, or new offences were recorded, but only sometimes and in different ways. This lack of consistency and the delayed nature of the updates, often rendered inaccurate information and made difficult to link these changes to other aspects of support.
The organisation also needed a more structured way to capture case notes and support sessions. Without consistent records of the type, duration, and impact of services, it was hard to demonstrate outcomes to funders or improve services.
Solution
Blaze Your Trail designed and implemented a solution built on the Salesforce Program Management Module (PMM). This extended Prison Network’s existing group program system into a comprehensive client and program management platform, tailored to individual women and their families.
To address life circumstances, Blaze Your Trail created a custom object with start and end dates, type, and location fields, along with automation that updates related records when circumstances change. This ensures accuracy and reduces manual data handling.
The team also developed a Sessions object to record one-on-one sessions and the services provided within.
The sessions have a customised layout with the supports available under each program and the selections are automatically converted into service delivery records. Indirect support sessions were also created to record this activity and all sessions have duration, channel, and notes. All this enables powerful reporting and dashboards.

The new system aggregates service delivery data across programs, enabling Prison Network to demonstrate its impact more clearly. Reports and dashboards provide evidence of outcomes to the Department of Health and other funders, supporting compliance and strengthening funding cases.
